Wellness Nurse - Full Time

Job DescriptionSalary: Starting at $32.75 per hourSchedule: Full Time, 2pm - 10pm, Every other weekend commitmentThe Wellness Nurse supports the health and well-being of residents through ongoing clinical monitoring and support, provides assistance to community nurses, caregivers, and medication personnel to help ensure regulatory compliance and clinical excellence. The ability to work effectively as part of a team with flexibility and adaptability to change and pace of duties while maintaining a warm and encouraging disposition is important.Assist in coordination of the resident admission process including screening, assessment, and care needsEnsure care is delivered as indicated in resident care plansMedication and treatment administration and ensure medications are administered per physicians’ ordersAssistance with coordinating care and services with other outside providers including resident physiciansDocument and report care provided and resident observations including unusual or significant changesMaintain a working knowledge of state regulations, policies, and procedures
