Advanced Clinical Practitioner

An exciting opportunity has risen for an ACP to join the site-wide practitioners team at the Broadgreen Hospital site. The post holder will be joining an established team of existing ACP's and medics to deliver high standards of care to inpatients primarily working within DMOPS (reablement) but could be deployed to any medical or surgical wards to provide general inpatient care.You will be supported within a multi- disciplinary team to provide excellent patient care using your expertise, knowledge and experience to this cohort of older patients. You will be part of MDT sessions to discuss best outcomes for our patients.You will be expected to contribute to audits and evaluation to support quality improvement and service development.The role requires occasional weekend working to ensure adequate medical cover to ensure safety within the site. You may also be required to undertake site manager responsibilities in the event of rota gaps. The shift pattern will follow a Monday-Friday 9-5 pattern but can be adapted to meet service needs.You will form part of the Medical Emergency Team and be expected to respond to emergency calls across the site.To improve the experience and outcomes of care for patients by taking the lead in investigations and treatments prescribed.To provide a consistent and personalised service by ensuring that thepatients receive the best care, in the best place from the best person or team.To provide social, emotional and psychological support for patients from referral and throughout their treatment.To deliver expert specialist evidence-based care including development, assessment, planning, implementing, evaluating programmes of evidence based care for a defined caseload.To make appropriate referral to consultants/other specialist/members of the multidisciplinary team in primary, secondary or tertiary care settings.To be aware of risk management issues both locally and nationally and develop strategies to address identified issues.Liverpool University Hospitals NHS Foundation Trust comprises Aintree University Hospital, Broadgreen Hospital & Royal Liverpool University Hospital.We are part of NHS University Hospitals of Liverpool Group, formed on 1 Nov 2024 from the coming together of LUHFT and Liverpool Women’s NHS Foundation Trust.
