The Housing Support Specialist is a highly engaged and mission-driven role that provides one-on-one support services to navigate housing opportunities, locations, and overcoming barriers with intentional, individualized service planning for adults with intellectual and developmental disabilities (IDD). Much like lesson planning, this role requires thoughtful preparation and execution of activities designed to build specific skills and support long-term personal development in both home and community settings.OverviewThe Housing Support Specialist promotes and coordinates housing stability activities to individuals with intellectual and developmental disabilities. The specialist coordinates with clients, landlords, and local community resources to develop housing plans and deliver independent living skills that promotes housing retention.Client Support and Case ManagementComplete intake assessments for new or progressing clients who are referred for housing services, ensuring all referral documentation is collected, reviewed, and validated for program enrollmentMaintain an active caseload to provide one-on-one support in identifying community resources, developing housing plans, and facilitating referrals and linkages as indicatedConduct comprehensive assessments across all domains of a client’s life to determine service needs and prioritiesProvide case management, including coordination and monitoring of person-centered plans (PCPs), and ongoing progress evaluationOffer individual supportive counseling and assist clients with symptom management, crisis stabilization, and goal attainmentEducate and support clients’ families, advocating for client rights, preferences, and self-determinationHousing Services and DevelopmentNavigate housing assistance programs including Section 8, HUD programs, HOPWA, and state-specific IDD housing initiativesConduct pre-inspections for new unit move-ins and relocations; coordinate with housing inspectors to ensure compliance with program and safety standardsObtain and document results of housing inspections and ensure all required follow-up actions are completedDevelop and identify housing opportunities within the community; maintain updated knowledge of available housing resourcesServe as a liaison and advocate between clients, landlords, and community partners to facilitate communication and problem resolutionProgram Implementation and CoordinationOrganize and conduct structured group and individual activities that support clients’ housing goals, community integration, and independent living skillsProvide direct service interventions aligned with the client’s PCP and in coordination with the multidisciplinary teamPerform field-based outreach and support—approximately 80% of work performed in community settingsDocumentation, Reporting, and ComplianceMaintain comprehensive documentation in compliance with agency policies and regulatory requirements, including housing plans, progress notes, person-centered planning (PCP) records, and all Medicaid, state, and federal documentation standards Conduct program compliance activities including audits, evaluations, and reporting to ensure adherence to program, agency, and funding source requirements Manage annual housing recertification process by obtaining, verifying, and maintaining all required documentation to ensure continued eligibility and compliance Administer financial management services for clients in the representative payee program, including preparation and oversight of monthly budgets Drive program excellence through active participation in annual service goal development, implementation tracking, and progress reporting against established metricsCollaboration and Community EngagementIdentify housing resource materials and procure them by agency proceduresRepresent the agency in community meetings and collaborative initiatives focused on housing and mental health servicesCoordinate transportation to clients for community meetings, inspections, or other necessary appointmentsProvide on-call crisis intervention support during evening and weekend hours as neededEducationMinimum an Associate degree with 5+ years of relevant experience in IDD services, case management, or housing supportExperience Experience with knowledge of affordable housing/rental assistance programs, landlord/tenant laws, Tax Credit ProgramsKnowledge of the (DDA) system, eligibility requirements, and related funding mechanismsBackground in providing on-site job coaching, workplace accommodations, and employment retention support is preferredCertifications & LicensesValid Maryland driver’s license with a clean driving record and reliable transportationMust have the DDA state approved certificate from Maryland Inclusive Housing (MIH)Compensation & Core BenefitsAnnual Salary: $60,000 – 70,000Comprehensive health, dental, and vision insuranceRetirement planPaid time off and holiday payMileage ReimbursementFlexible scheduling to balance housing services responsibilitiesPreferred Additional QualificationsBilingual capabilitiesBackground in disability advocacy or servicesExperience with assistive technology and communication devicesKnowledge of evidence-based practices in skill development and community integrationAllied Community Care Inc. is an equal opportunity employer and values diversity.We are committed to creating an inclusive environment for all employees and individuals served.Powered by JazzHREsd0aHZCvv
