Role Description The Social Worker will provide direct support and advocacy to clients by assessing their needs, creating care plans, and connecting them to community resources. Day-to-day responsibilities include conducting home visits, maintaining accurate records, delivering therapeutic interventions, and collaborating with other health professionals to ensure holistic care. This is a full-time, on-site role located in Hobart, TAS. Qualifications Client assessment, case management, and care plan development skillsKnowledge of community resources and social services networksStrong interpersonal communication and advocacy skillsProficiency in maintaining accurate and confidential documentationExperience in therapeutic interventions and counseling practicesDegree in Social Work or equivalent qualificationCurrent registration with a recognized Social Work governing bodyAbility to work collaboratively as part of a multidisciplinary teamExperience in aged care, mental health, or disability services is an advantage
