Role Description This is a part-time hybrid role for a Support Coordinator based in Springfield, QLD, with some flexibility to work from home. The Support Coordinator will assist NDIS participants in navigating their plans, coordinating various supports, and connecting them with appropriate services. Responsibilities include building strong relationships with service providers and stakeholders, maintaining accurate case records, providing expert advice, and ensuring that participants' needs are met in line with their goals. The role also requires regular follow-up with clients to ensure continued support and progress. Qualifications Strong Interpersonal and Communication skills to build relationships with clients, service providers, and stakeholders effectively.Experience in Case Management and Customer Service, ensuring client needs are met and providing excellent support throughout the process.Proficiency in Technical Support or familiarity with related tools and platforms is an advantage.Ability to work independently while maintaining collaboration with a team.Knowledge of the NDIS framework and prior experience working with individuals with diverse needs is a plus.A qualification in Social Work, Community Services, or a related field is highly desirable.
